Aged Care Support Specialist

Job OverviewWe are currently seeking a compassionate and caring individual to join our team as an Aged Care Support Specialist. This role will involve providing exceptional care and support to our residents, ensuring their dignity and well-being are maintained at all times.
Key Responsibilities- Deliver personal care and assistance to residents in accordance with their individual needs and care plans
- Assist with medication management, including using medication compliance aids
- Monitor and respond to changes in resident health and wellbeing
- Provide wound care and implement continence programs as required
- Conduct routine checks, including blood pressure, temperature, pulse, and blood sugar
- Assist residents with diabetes management
- Maintain accurate documentation throughout each shift
- Contribute to the development of resident care plans
- Certificate III or IV in Aged Care and current First Aid Certificate
- Experience assisting with medication
- An understanding and commitment to person-centred care
- Knowledge of WHS and infection control principles
- An understanding of resident changing care needs and AN-ACC reviews
- Computer literacy and experience with documentation
- Excellent communication skills
- A commitment to continuous improvement
- Ability to work in a team environment
- National NDIS worker screening check
