We are currently seeking a highly skilled Care Coordinator Specialist to join our team in the Rockhampton office. This permanent part-time role offers the ideal opportunity for a Registered Nurse or Enrolled Nurse to utilize their expertise and passion for high-quality client-focused care in a community setting.

The successful candidate will possess excellent communication skills, be able to work autonomously and collaboratively as part of a dynamic team, and demonstrate a commitment to ongoing professional development. They will also have a strong ability to interpret and implement health, aged, and disability care policies and regulations, ensuring compliance with local, state, and federal regulatory agencies.

A key aspect of this role will be to develop and maintain effective relationships with clients, caregivers, and other healthcare professionals, ensuring seamless coordination of care services. The successful candidate will also be responsible for implementing and maintaining operating systems, including Visual Care, and ensuring that all relevant policies and procedures are up-to-date and compliant.

We offer a competitive salary package, estimated to be around $65,000-$85,000 per annum, depending on experience. The ideal candidate will have:

  • Current registration with AHPRA
  • Satisfactory Australian National Police check
  • Current or willing to obtain a first aid and CPR certificate
  • Professional Indemnity Insurance

  • Driver's licence and reliable presentable car
  • COVID-19 vaccinations
