Job Description

Role Overview: We are seeking a dedicated and compassionate Care Coordinator to join our team. As a key member of our care coordination team, you will be responsible for delivering high-quality in-home support services to our clients. This role involves service development, coordination, and delivery, including ongoing client assessments and the development of personalized care plans.

Key Responsibilities:

  • Conduct thorough client assessments to identify their needs and develop tailored care plans
  • Coordinate and deliver in-home support services to ensure seamless care delivery
  • Provide direct nursing support to clients as required
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
  • Maintain accurate records and reports to ensure compliance and quality assurance

Required Skills and Qualifications:

  • Diploma in Nursing with current AHPRA registration
  • Driver's License and comprehensively insured vehicle
  • Current National Police Clearance and First Aid & CPR certification
