Empowering Social Care Professional

We are seeking a dedicated and passionate professional to join our team as an Empowering Social Care Practitioner. As a key member, you will provide individualized support within a strengths-based model, working towards empowering clients to reach their full potential.

The ideal candidate will have a degree in Social Work and eligibility for practicing membership of the relevant professional association. You will be committed to advocating for individuals at any stage of their life, with a neuroaffirming perspective based on the social model of disability.

Responsibilities will include:

  • Maintaining accurate records, data, and information, using established policies and procedures
  • Completing Functional Capacity Assessments, ensuring adherence to relevant standards and best practice
  • Working with clients in a one-to-one or small group capacity, using a variety of therapeutic techniques
  • Coordinating support services to meet client needs
  • Developing and implementing individualized plans to achieve positive outcomes
