Job Description

This role involves working with family members to develop a tailored plan and connecting them to the services that best meet their needs.

The successful candidate will be employed by Redkite but will work within the Cancer Hub and will predominantly focus on supporting Redkite families.

The Cancer Navigator will advocate for the meeting of those needs and assist with coordinating the actions of multiple treatment and support providers.

Key Responsibilities:
  • Provide high-quality, evidence-based cancer navigation support to young people and families impacted by cancer.
  • Actively participate in psychosocial review meetings, communicating changes, challenges and needs associated with each service user and their family.
  • Utilize a case management approach with practical support methods to facilitate referrals and other supports.
  • Identify and contribute to key stakeholder relationships to ensure young people and families receive access to the support that best meets their needs.
  • Collaborate and consult with peers, share practice through teaching and learning opportunities.
  • Contribute to team projects and overall development of the service.
Required Skills and Qualifications:
  • Tertiary qualification in Social Work.
  • Demonstrated experience in delivering case management and/or clinical intervention within child, youth or health focused settings will be highly regarded.
  • Experience creating and maintaining relationships with sector partners such as hospitals, health and community services, education and not-for-profit professionals.
  • Experience in conducting risk assessments and developing safety plans.
  • Knowledge of child protection legislation and mandatory reporting.
  • Experience with working in a multi-disciplinary team, liaising with staff from different departments (internal and external) and working within a remote team.
  • Demonstrated skills in decision making, problem solving, negotiation and crisis management.
  • Outstanding communication skills and a natural ability to connect.
