Role Overview

This opportunity enables you to join our organisation as a NDIS Support Coordinator. This position ensures effective customer service delivery by coordinating supports to NDIS participants, consistent with organisational policies, across metropolitan and/or regional locations.

Key Responsibilities:
  • Ensure person-centred approaches are understood and incorporated into NDIS support plan implementation and service offerings/supports.
  • Implement individual plans and ensure alignment of person-centred approaches based on relevant practical and theoretical knowledge.
  • Contribute to planning and development of service offerings/supports.
  • Address complex barriers affecting individuals' ability to access suitable supports.
  • Design a service plan for a person's specific support needs, and specialist support coordination where necessary.
  • Ensure participants are appropriately supported in exploring and resolving their needs, expectations and goals.
Skills & Experience:
  • Experience in coordinating service provision in the disability or related sectors.
  • Significant experience working in human services settings and knowledge of various intervention models.
  • Specialist knowledge of disability and relevant support services.
  • Ability to provide advice and facilitate NDIS participants' planning to meet objectives.
  • Ability to promote the service and client group issues to other organisations, government bodies, and community groups.
  • Experience working with multi-disciplinary teams.
  • Advocate on behalf of the Participant and mediate/resolve conflicts when Participants have issues with service providers.
  • Coach and assist with Participant skill development.
