About the role
Pathway Links is seeking a Level 2 Support Coordinator to provide services under the National Disability Insurance Scheme (NDIS). In this role, you will support people with disabilities in navigating the NDIS and accessing the services and supports they need to lead independent and fulfilling lives.
What you'll be doing
- Coordinating and managing the delivery of support services to participants
- Liaising with participants, their families, and a range of service providers to ensure their needs are met
- Developing and implementing individualised support plans to help participants achieve their goals
- Working collaboratively with participants and service providers to ensure access to appropriate services and funding
- Monitoring and reviewing participant progress, and making adjustments to support plans as needed
- Maintaining accurate records and documentation in accordance with organisational and regulatory requirements
- Participating in team meetings and professional development activities
What we're looking for
- Relevant education or certification in disability or a related field such as social work or community services
- Experience in a similar support coordination or case management role, preferably in the aged care or disability sectors
- Strong communication and interpersonal skills, with the ability to build positive relationships with participants, families, and service providers
- Excellent problem-solving and decision-making skills, with the ability to work autonomously and as part of a team
- Proficient in using relevant technology and software to manage records and support plans
- Understanding of the NDIS and other government support programs and funding
- Commitment to upholding the rights and promoting the independence of people with disabilities
