About the Role
As a Case Manager, you will oversee and coordinate the onboarding process of new Home Care Package clients in the Central South Metro region. Your responsibilities involve developing relationships with local networks and external agencies to enhance access to services and supports for daily living.
You will work closely with clients to identify their ongoing needs and goals, ensuring they remain in their own home. Key tasks include monitoring client budgets, managing reports, and maintaining high-quality information and support as requested by the Regional Home Care Manager.

About You
We are seeking an experienced Case Manager with knowledge of case management principles, Home Care Package guidelines, and funding body experience. Essential skills include excellent organisational abilities, the capacity to manage multiple priorities, and strong communication skills. A valid driver's licence and access to a reliable vehicle are also required.
