Client Service Coordinator

As a Client Service Coordinator, you will be responsible for assessing, planning and coordinating tailored services for older individuals.
This role involves working as part of a multidisciplinary team to support clients in optimising their independence and quality of life in their own homes.
The successful candidate will possess significant experience in a health, aged or community service environment, with an understanding of home care packages, CHSP guidelines and Aged Care Quality Standards.
Excellent communication and interpersonal skills are essential for this role, as you will be required to inclusively communicate with a diverse range of people.
You will also need to demonstrate the ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
Key responsibilities include:
- Assessing client needs and developing personalised service plans
- Coordinating services with other healthcare professionals
- Providing emotional support and advocacy for clients
- Maintaining accurate records and reports
The ideal candidate will have:
- Significant experience in a health, aged or community service environment
- Excellent written and verbal communication skills
- Strong customer service focus and person-centred skills
- Demonstrated ability to work effectively in a team environment
