Job Overview:

We are seeking a skilled Coordinator to support individuals across the region, enabling them to understand and implement their NDIS plans.

Key Responsibilities:
  • Assist participants in understanding and implementing their NDIS plan by providing education and guidance.
  • Coordinate services and supports to meet individual goals, ensuring effective delivery and efficient use of resources.
  • Develop participants' capacity to manage their own supports over time, promoting independence and self-advocacy.
  • Liaise with service providers, allied health professionals, and community supports to ensure seamless coordination and collaboration.
  • Monitor and track progress, identifying areas for improvement and making recommendations for enhanced service delivery.

This role is ideal for someone who is highly organised, empathetic, and able to work independently while building strong relationships with participants and their care teams.
