Help at Hand Support Services is a dedicated provider of Support Coordination services, committed to making a difference in the lives of NDIS participants across Victoria.
Salary InformationWe offer a competitive salary package of AU$60,000 - AU$80,000 per annum, depending on experience and qualifications.
Job DescriptionThe Support Coordinator is responsible for providing personalized support to clients of the National Disability Insurance Scheme (NDIS). This role involves working closely with clients to implement supports outlined in their NDIS plans, fostering capacity building, managing the impact of their disabilities, and enhancing their overall health, well-being, and community participation.
Key Responsibilities:- Establishing connections with key stakeholders, including NDIS Providers, community venues, and healthcare teams.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Minimum Certificate IV in Disability or related field.
- Demonstrated knowledge and experience of NDIS.
- Strong interpersonal, written, and verbal communication skills.
- Ability to develop good working relationships with internal and external stakeholders, including participants, families, and health providers.
- Previous Support Coordination or Case Management experience.
- Australian Driver's license and vehicle.
- NDIS Worker Screening check.
- Working with Children Check.
