Key Responsibilities:- Provide direct care services to individuals who are aged, frail, or have disabilities.
- Develop individualized care plans that promote independence and well-being.
- Work collaboratively with healthcare professionals, families, and carers to ensure comprehensive support.
- Conduct regular assessments and evaluations to ensure the effectiveness of care plans.
- Completed Certificate III or higher in Individual Support (Aged Care, Disability, or Home and Community).
- Proven experience in exercising judgment and initiative in a day-to-day role.
- Experience in providing direct care services in a health-related field.
- Experience in dealing with aged, frail, or disabled persons.
- Proficiency in using computer packages, such as Microsoft Word and Excel.
