As a Community Support Worker, you will provide personalized support services such as personal care, medication management, cleaning, meal preparation, transportation, and social support.
About the Role:
- Provide direct care services to individuals in their own homes, promoting independence and well-being.
- Develop and implement personalized care plans that meet the unique needs and goals of each individual.
- Work collaboratively with consumers, families, and other healthcare professionals to ensure seamless care coordination.
- Maintain accurate and up-to-date records of client interactions and progress.
Requirements:
- A minimum of 2 years' experience in providing direct care services to individuals with complex needs.
- Completed Certificate III or higher in Individual Support (Aged Care, Disability, or Home and Community).
- Proven ability to work autonomously and as part of a team, exercising sound judgment and initiative.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues.
