Social Work Specialist

The role of a Social Work Specialist is critical to the success of our team. This position will involve coordinating discharge planning, conducting thorough psychosocial assessments, and providing support, guidance, and evidence-based intervention strategies to enhance social functioning.

  • Coordinate discharge planning in collaboration with multidisciplinary teams.
  • Conduct comprehensive psychosocial assessments of patients attending the hospital.
  • Provide support, guidance and other evidence-based intervention strategies to enhance social functioning.
  • Maintain high standards of written communication, with accurate and timely medical/clinical record entries.
Requirements
  • Demonstrated ability to effectively prioritise and organise own work/caseload.
  • Ability to liaise with and maintain effective links with relevant external organisations.
  • Commitment to evidence-based practice and ongoing personal and professional development.
  • Bachelor of Social Work or other equivalent qualification and eligible for membership of the AASW.
