Job Summary

As an Aged Care Coordinator, you will play a vital role in managing the service for new Commonwealth Home Support Program (CHSP) clients. You will be responsible for ensuring all inquiries are addressed promptly and professionally, assisting with the intake of clients and scheduling of services, and taking on a leadership role to a team of Home Care Assistants.

The successful candidate will have a Certificate IV in a relevant industry qualification and/or significant industry experience, as well as current unencumbered Driver's Licence, current first aid and CPR certification, and a satisfactory Criminal Record Check.

You will also have the ability to interpret and work within relevant program guidelines, monitor service provision, mentor and share knowledge with less experienced team members, and manage a complex workload through planning and organisation of priorities.

Key Responsibilities:
  • Manage CHSP client services
  • Assist with client intake and scheduling
  • Lead a team of Home Care Assistants

Requirements:

  • Certificate IV in a relevant industry qualification
