Key Responsibilities- Provide mobile, outreach, and centre-based case management support to individuals and families experiencing homelessness
- Conduct holistic assessments of client needs and develop case plans to promote social inclusion, human rights, participation, and consumer choice
- Work collaboratively with staff and clients from diverse backgrounds to provide culturally sensitive and respectful services
- Develop and maintain relationships with community service providers to ensure seamless referrals and support
- Monitor and evaluate the effectiveness of case management services and make recommendations for improvement
- Sound casework and case management experience, including holistic assessment of client needs and development of case plans
- Demonstrated ability to work directly with clients at risk of homelessness who have complex needs
- Experience in working competently with staff and clients from cultural and linguistically diverse backgrounds
- Well-developed interpersonal and written communication skills
- Working with Children Check (Blue Card)
