As a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different to other support providers.

The successful candidate will be responsible for creating and continuously developing relationships between participants and their team of Hearth Support Workers, assessing participant needs, and developing Hearth's support program.

Key responsibilities will include:

  • Building and maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders
  • Leading the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals
  • Reporting on the progress of clients, as it relates to the support provided by Hearth, and preparing submissions for NDIS plan reviews as required
  • Coordinating a weekly staff roster for each participant and proactively resolving rostering issues
  • Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan

Requirements:

  • Proven experience working in the disability sector, preferably in a supervisory or managerial role
  • A Professional level of administrative and business communication skills, including case noting and report writing
  • Excellent time management and organisational skills
  • Proven client centred approach with the ability to advocate for Participants and an understanding of complex disability and trauma
  • Own car and driver's license
  • Strong interpersonal skills and high levels of resilience
  • Understanding of the NDIS
  • Working knowledge of Microsoft office suite and employee rostering systems
