We are seeking a dedicated and compassionate Support Worker to join our team at Hearth Support Services.

The successful candidate will have experience supporting individuals with disabilities, with a focus on providing person-centred care and promoting independence.

The role will involve:

  • Assisting with daily living tasks, such as personal care and meal preparation
  • Supporting participants with community access and recreational activities
  • Providing transportation and social engagement opportunities
  • Collaborating with the support team to develop and implement individualised support plans

Requirements

We are looking for a candidate with:

  • Professional experience supporting individuals with disabilities
  • Strong communication and interpersonal skills
  • A valid driver's license and reliable vehicle
  • NDIS Worker Screening Check
  • The legal right to work in Australia
