Hearth Support Services is a NDIS Registered Service Provider seeking a skilled Relationship Manager to join our team at our Bendigo office.

We strive to create an environment that nurtures, supports, and engages with our participants and support workers to consistently provide the highest standard of care.

About the role:

The Relationship Manager will be the central point of contact for support workers and participants, ensuring a clear and consistent point of contact.

Key responsibilities include:

  • Developing and maintaining relationships between participants and their team of Hearth Support Workers
  • Assessing participant needs and developing Hearth's support program
  • Maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders
  • Leading the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals
  • Reporting on the progress of clients and preparing submissions for NDIS plan reviews as required
  • Coordinating a weekly staff roster for each participant and resolving rostering issues
  • Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan
  • Compliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reporting

Requirements:

  • Proven experience working in the disability sector, preferably in a supervisory or managerial role
  • Professional level of administrative and business communication skills, including case noting and report writing
  • Excellent time management and organisational skills
  • Proven client centred approach with the ability to advocate for Participants and an understanding of complex disability and trauma
  • Own car and driver's license
  • Strong interpersonal skills and high levels of resilience
  • Understanding of the NDIS
  • Working knowledge of Microsoft office suite and employee rostering systems
