Community Rehabilitation Nurse

The South Eastern Sydney Local Health District is seeking a skilled Community Rehabilitation Nurse to provide comprehensive assessments and case management to older people returning home after discharge from hospital.

Job Responsibilities:
  • Conduct nursing assessments in clients' homes and collaborate with the allied health team to develop individualized care plans that promote independence and optimal health outcomes.
  • Provide education and support to clients and their families/carers regarding long-term services for ongoing safe and appropriate care.
  • Collaborate with healthcare providers to ensure seamless care transitions and maximize rehabilitation outcomes.
Key Requirements:
  • Registration with AHPRA as a Registered Nurse/Midwife.
  • High-level written and verbal communication, interpersonal skills, and ability to identify key messages, issues, and concerns when communicating with others.
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team, applying professional and ethical boundaries when dealing with complex situations.
  • Computer skills, including data collection, email, and Microsoft Word, and proven ability to use health information systems.
  • Detailed knowledge and understanding of risk management, safety, and quality principles, and the role of the Registered Nurse in applying these principles.
  • Recent experience as a Registered Nurse in aged care and/or rehabilitation, and current professional practice knowledge related to working in the community as a Registered Nurse.
