The role of Geriatric Care Coordinator is a rewarding opportunity for Registered Nurses to make a genuine difference in the lives of older people. This position enables professionals to grow or transfer their nursing skills, supporting individuals to live well in the comfort of their own home.
As a Geriatric Care Coordinator, you will coordinate the delivery of high-quality care and services that empower our customers to remain living at home. You will develop genuine connections with our customers and loved ones, assist them to implement innovative strategies that optimise their health and wellbeing, and support independence.
The role involves managing a caseload of clients and being responsible for the clinical care/governance. You will set individual goals that are realistic and achievable, and implement the latest best practice and evidence-based approaches.
You will also provide guidance and clinical support to Care Facilitators caring for our Home Care Package clients.
This position is home-office based with support from a local site office.
Requirements:
- An Australian driver licence and reliable motor vehicle with Third Party Property Damage car insurance, as travel to clients' homes is required
- Registered Nurse qualifications with current AHPRA registration
- Proven experience working with budget parameters and service delivery
- Demonstrated ability to lead and work as part of a team
