About Us

We work collaboratively with young mothers and their children to provide a nurturing environment that promotes positive outcomes. By offering individualised and community therapeutic interventions, we empower families to overcome adversity and achieve self-sufficiency.

Your Responsibilities

As a Therapeutic Case Manager, you will lead a multidisciplinary team in delivering best-practice case management, individualised and community therapeutic interventions, and leadership of Therapeutic Carers. You will support the development of young mothers and their children through case planning, goal setting, and regular review and update of plans.

Key Skills and Qualifications
  • Certificate IV in Child, Family and Youth Intervention or related qualifications
  • Experience in a care-oriented role, preferably in working with young people who are experiencing or at risk of homelessness
  • Excellent communication and interpersonal skills, enabling effective collaboration with colleagues, young people, and families
