About the Role

As a Disability Support Specialist, you will work closely with participants, their families, and support teams to develop person-centered plans that promote independence, empowerment, and inclusion.

You will be part of an innovative team that utilizes evidence-based practices and social model therapy to break down barriers and achieve high-impact outcomes for people with disabilities.

Key Responsibilities
  • Conduct initial assessments using our LEAD evidence-based assessment tool to identify participant needs and goals.
  • Liaise with the interdisciplinary team to provide wraparound support and ensure seamless delivery of services.
  • Support participants in their everyday environments, promoting capacity building and skills development.
  • Prioritize risk management while ensuring safeguards are in place to protect vulnerable individuals.
  • Engage in regular peer supervision, mentoring, and professional learning to enhance expertise and skills.

Requirements

To be successful in this role, you must:

  • Hold a Bachelor or Masters degree in Disability (or equivalent) and be eligible for DEAI membership.
  • Obtain an NDIS Worker Screening Check & Working with Children's Check.
  • Hold a valid driver's license.
