About the Role

The Support Coordinator plays a vital role in strengthening the ability of Aboriginal NDIS and MyAgedCare participants to access and coordinate supports in a culturally appropriate manner. This involves working collaboratively with other service providers to ensure services are delivered in accordance with the NDIS Practice Standards, National Standards for Disability Services, and Aged Care Standards.

Key Responsibilities

* Coordinate supports with an emphasis on client goals and making the best use of funded hours
* Develop and regularly review progress of individual plans and implementation of participants plans
* Ensure services are delivered in accordance with relevant standards and models

About You

To be successful in this role, you will have a minimum Certificate IV in Community Services or equivalent and/or at least 2 years of relevant work experience. You will also have demonstrated skills and knowledge in care coordination, including planning, implementing, reviewing, and evaluating effective service plans with an emphasis on client goals. Additionally, you will have a sound knowledge and experience of NDIS Coordination and the ability to communicate in a culturally appropriate manner.
