Community Liaison

Overview

  • This is a role that involves developing and nurturing relationships with referral sources such as hospitals, physician offices, skilled nursing facilities, senior living communities, social workers, and discharge planners.
  • Promote FirstLight's services through consistent in-person outreach, presentations, and local community engagement.
  • Identify, follow up on, and convert leads into active home care clients.

Key Responsibilities

  • Develop strong partnerships with referral sources to generate new client opportunities and support the growth of our local presence.
  • Promote FirstLight's services through targeted outreach, presentations, and community engagement efforts.
  • Work closely with the care coordination team to ensure a smooth and responsive onboarding experience for clients and their families.
  • Meet and exceed monthly sales and growth targets aligned with agency goals.

Required Skills and Qualifications

  • 2+ years of outside sales or business development experience in home care, hospice, healthcare, or a related field.
  • Demonstrated success meeting or exceeding sales targets.
  • Excellent communication, relationship-building, and public speaking skills.
  • Self-motivated, highly organized, and comfortable working independently.

Work Location

  • In the field Northeast Cleveland (eastside of Cuyahoga County, Geauga and Lake Counties) with some in-office responsibilities.
