Job Opportunity

We are seeking a seasoned leader to spearhead our Dementia Centre initiative, supporting individuals transitioning from hospital settings back to their homes or into residential care.

Key Responsibilities

  • Provide strategic leadership to a multidisciplinary team across multiple locations, focusing on delivering high-quality services in accordance with established policies and procedures.
  • Collaborate with an integrated team to ensure a consistent approach to service delivery, leveraging expertise and resources to drive excellence.
  • Foster a culture of empowerment, mentorship, and support among designated team members, prioritizing their wellbeing and safety.
  • Oversee individual and team performance, ensuring the highest standards of quality and case management within the DSA Service Delivery framework.
  • Contribute to achieving administrative, financial, risk, and quality goals for the DSA Program, driving operational efficiency and effectiveness.
  • Offer direct consultation and support to clients as needed, tailoring services to meet unique needs.
  • Ensure timely referral allocation to relevant staff members, aligning with established KPIs.
  • Recruit skilled professionals to join the team, matching talent with role requirements.
  • Effectively manage stakeholder relationships through strategic conversations, fostering collaboration and open communication.
  • Address customer complaints and challenging situations promptly and professionally.
  • Pursue ongoing professional development as part of the Learning and Dementia Centre team.
