We are seeking a Team Leader to join our Dementia Centre initiative, supporting people living with dementia who are transitioning from hospital settings back to their homes or into residential care. The Team Leader ensures that the Dementia Support Australia service is tailored to the needs of individual clients and ensures the service delivers on its key performance indicators. Responsibilities include providing leadership to a multidisciplinary team, developing a culture of empowerment and support, oversight of individual and team performance, and contributing to ensuring the wider DSA Program meets its administrative, financial, risk and quality and reporting goals.
Key responsibilities also include fostering collaboration and communication among team members, promoting a positive work environment, and managing resources effectively to achieve program objectives.
