We are pleased to share this opportunity to be involved in a centre initiative looking to support individuals living with dementia who are transitioning from hospital settings back to their homes or into residential care.

Key Responsibilities:- Provide leadership to a multidisciplinary team of staff across multiple locations, with a specific focus on the programme, in accordance with our policies, procedures and values.
- Work as part of an integrated team to ensure a nationally consistent approach to our 'Next Chapter'.
- Develop a culture of empowerment, mentorship and support to the designated team of professionals, ensuring their wellbeing and safety is considered.
- Oversight of individual and team performance to ensure delivery operates within the case management framework, of the highest quality, to reduce the impact of behaviours on individuals living with dementia, their carers and staff.
- Working in partnership with other team leaders to meet the needs of the broader programme and contribute to solutions to any broader deliverables.
- Contribute to ensuring the wider programme meets its administrative, financial, risk and quality and reporting goals across the programmes' KPIs.
- Provide direct consultation and support to clients as required;
- Ensure timely allocation of incoming referrals to the appropriate staff member in alignment with programme KPIs.
- Provide regular reporting to the regional manager on all staff performance, service quality, operational performances and issues.
- Recruit professionals with appropriate skills for the relevant role.
- Manage stakeholder relationships through effective strategic conversations with local managers and staff in residential aged care, home care and the hospital settings to ensure that the programme is understood and key relationships are effective.
- Effectively manage customer complaints or challenging situations as they arise, with early notification to the regional manager when needed.
- Work with the regional manager to address incidents and feedback relating to the quality of the programme.
- Continuously improve knowledge and skills as part of the learning and development team.
- A commitment to align with the organisation's mission, motivation and action.
- Tertiary qualifications in Nursing/Allied Health or health-related degree.
- Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.
- The ability to manage and support a team of skilled professionals across multiple sites and locations.
- The ability to work on multiple tasks and projects simultaneously.
- High-level communication and interpersonal skills.
- Strong planning and organising skills with the ability to prioritise tasks to meet deadlines.
- Strong analytical skills and attention to detail.
- Good customer management skills for delivery of services to clients.
- Strong administrative and computer skills.
- A willingness to travel intra and interstate.
