About the Role

We are seeking a skilled Aboriginal Health Practitioner to join our Integrated Chronic Care for Aboriginal People Program.

The successful candidate will be responsible for providing culturally safe and patient-centered services to Aboriginal clients and their families with chronic or complex needs.

This role requires a motivated and enthusiastic individual who can work in partnership with a Clinical Nurse Consultant and as part of a collaborative team.

Key Responsibilities

  • Provide assessment, care coordination, and education to clients, families, and carers considering clinical, psychosocial, and cultural needs.
  • Work collaboratively with the Clinical Nurse Consultant and broader team to support and coordinate care.

Requirements

  • Degree in a relevant health-related field (e.g., nursing, medicine).
  • Experience working with Aboriginal communities and populations.
  • Strong communication and interpersonal skills.
