Healthcare Specialist for Chronic Care

An exciting opportunity exists for an experienced Aboriginal Health Practitioner to join our Integrated Chronic Care for Aboriginal People Program. The successful candidate will be responsible for providing culturally safe, patient-centered services to Aboriginal clients and their families with chronic or complex needs.
This position requires a motivated, enthusiastic, and conscientious Aboriginal Health Practitioner who has the ability to work in partnership with a Clinical Nurse Consultant and as part of the broader team. The successful applicant will be supported by the Integrated Chronic Care Clinical Nurse Consultant, their Line Manager and the broader ICCAPP and Aboriginal Chronic Care Program Teams.
We are looking for someone with excellent communication skills and the ability to build strong relationships with clients, families, and healthcare professionals. The ideal candidate will have experience working with Aboriginal communities and be familiar with cultural safety principles and practices.
About the Role:
The successful candidate will be responsible for:
- • Providing assessment, care coordination, and education to clients, families, and their carers that considers clinical, psychosocial, and cultural needs
- • Working closely with a Clinical Nurse Consultant to provide high-quality care to Aboriginal clients and their families
- • Collaborating with other healthcare professionals to ensure comprehensive care plans are developed and implemented

Key Requirements:
To be considered for this role, you must have:
- • A Bachelor of Health Science or related field, and/or equivalent qualifications and experience
- • Experience working with Aboriginal communities and understanding of cultural safety principles and practices
- • Strong communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals
