Job Overview\

We are seeking a dedicated individual to fill the role of Care Coordinator.

\

As a Care Coordinator, you will be responsible for coordinating and managing the delivery of care services to our clients. This includes working collaboratively with clients to coordinate supports and services that improve their quality of life.

Key Responsibilities

  • Deliver high-quality outcomes and goals for clients based on their care plans\
  • Conduct thorough client care assessments and develop personalized care plans in collaboration with clients and other stakeholders\
  • Supervise and manage care staff to ensure high-quality service delivery\
  • Coordinate the provision of quality person-centered care according to client needs and available funding\
  • Build and maintain strong relationships with local stakeholders and community partners\
  • Schedule appointments and in-home services with care staff as needed\
  • Ensure compliance with relevant government regulations and standards\

Requirements

  • \
  • Current registration as an Enrolled Nurse (EN) or Registered Nurse (RN) with AHPRA\
  • Certified III or IV in Aged/Disability Care with minimum 4 years of experience\
  • Proven experience working in community settings, particularly in aged care and disability\
  • Strong understanding of aged care and disability standards, including consumer-directed care principles\
  • First Aid certification, current CPR skills, and National Police Check\
  • WWC clearance or willingness to obtain\
  • Valid Australian Driver's License\
  • High level of computer literacy\
  • Understanding of clinical governance and continuous improvement\
