Small home care services provider seeks experienced professional to coordinate packages and support clients.

**Job Description:**

- Coordinate delivery of person-centred home care services, promoting healthy ageing and community re-engagement
- Liaise with ACAT Assessors and conduct initial visits with potential new clients
- Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
- Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options
- Provide leadership, support and mentoring to the support staff
- Be involved in identifying gaps and creating continuous improvement initiatives

**Requirements:**

- Bilingual and able to speak English and Italian is essential
- Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
- Able to create and review care plans that match client's needs and goals
- Committed to using a client directed model of care to empower the elderly and encourage reablement and community engagement
- A current driver's license and the ability to manage your time and work independently
