About this role:
">- A Care Navigator is required to work collaboratively with consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.
The successful candidate will deliver practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time.
">Key Responsibilities">- Facilitate consumer-centred care by connecting them with relevant services and supports.
- Provide information and education to consumers to support their care needs.
- Identify and address barriers to access, and facilitate warm referrals to ensure consumers receive the right support at the right time.
- A tertiary qualification in a relevant discipline, and/or extensive experience in mental health, health, community services or social sciences.
- Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
- Demonstrated understanding of supports and services available in the local and regional communities.
- Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
- Current driver's license and willingness to travel for the role.
