Health Support Specialist

This is a collaborative role where you will work with clients, healthcare providers, and community services to provide holistic support.
About the Role- You will implement care plans and connect clients to vital health and social services.
- Craft personalized support plans that enhance health and well-being, re-establishing connections to community groups and essential services.
- Tackle broader challenges affecting client's health, such as financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.
- Provide tailored guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices.
- You should have relevant tertiary qualifications in a field such as nursing, allied health, or social work.
- You should have a solid understanding of the determinants of health and their impact on communities.
- You should have experience in co-developing person-centred support plans that prioritize client well-being and aspirations.
- You should have advocacy skills to enhance client access to programs and inspire community participation.
- You should have exceptional communication and interpersonal abilities, particularly with culturally and linguistically diverse backgrounds.
- You should have experience in partnership building and collaborative work across various organizations to achieve client-focused outcomes.
