About the Role

The position of SAPHS Aboriginal and Torres Strait Islander Health Practitioner is a key part of our efforts to improve health outcomes for Aboriginal patients in South Australia. This role will work within the prison setting to implement and embed culturally safe care, reduce disengagement with treatment and care plans, and support hospital avoidance.

  • Provide clinical intervention, assessment, care planning, and education to Aboriginal patients, including those with complex needs.
  • Assist multidisciplinary team members on culturally appropriate interventions.
  • Support hospital avoidance and provide sub-acute health responses.
Key Responsibilities
  • Implement and embed a stronger and more sustainable approach to support Aboriginal patients that are incarcerated.
  • Deliver culturally safe care through an 'Aboriginal lens' to healthcare.
  • Work collaboratively with multidisciplinary teams to assess and provide clinical intervention to Aboriginal patients.
Requirements

To be successful in this role, you will need:

  • Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) or equivalent.
  • Eligibility for registration with the Aboriginal and Torres Strait Islander Health Practice Board of Australia (part of AHPRA).
  • Working towards or hold an appropriate Skill Set in the designated discipline.
