The Central Adelaide Local Health Network is seeking a dedicated and passionate Aboriginal and Torres Strait Islander Health Practitioner to join our Acute and Urgent Care program at The Queen Elizabeth Hospital Emergency Department. You will work in a multidisciplinary team to embed an 'Aboriginal lens' into acute healthcare settings, ensuring Aboriginal patients receive the care and support they need to achieve better health outcomes.

You will play a critical role in enhancing cultural safety, reducing the risk of disengagement, and improving continuity of care through appropriate referrals, discharge planning, and ongoing support. Your expertise will contribute to closing the gap in Indigenous health outcomes by reducing hospital avoidance, supporting timely and effective care, and promoting health equity.

This role offers a chance to develop your skills in providing culturally safe and holistic care to Aboriginal patients in acute healthcare settings. Under the guidance of experienced clinicians, you will support patients presenting to hospital, attending outpatients, or engaging with other care programs by ensuring their healthcare journey is culturally appropriate, supportive, and well-coordinated.

Key Responsibilities:

  • Provide culturally safe and holistic care to Aboriginal patients in acute healthcare settings.
  • Work collaboratively with multidisciplinary teams to reduce hospital avoidance, support continuity of care, and improve health outcomes for Aboriginal communities.
  • Develop your skills in providing culturally safe and holistic care under the guidance of experienced clinicians.
  • Support patients presenting to hospital, attending outpatients, or engaging with other care programs by ensuring their healthcare journey is culturally appropriate, supportive, and well-coordinated.
