**Case Manager Role Opportunity**
We are seeking a skilled Case Manager to join our team in supporting individuals and families affected by domestic violence, family violence, sexual violence, and child sexual abuse. This is a challenging yet rewarding role that requires strong case management skills, a deep understanding of trauma-responsive practice, and the ability to work with clients through all stages of engagement.
As a Case Manager, you will be responsible for providing effective case management to adults, young people, and children who have experienced mental health concerns due to traumatic events. You will work collaboratively with clients to develop, implement, monitor, and review their plans, identifying strengths and areas of vulnerability to ensure they are actively engaged in the process.
**Key Responsibilities**
- Intake triage and assessment
- Providing case management to support recovery from mental health concerns related to trauma
- Engaging clients using strengths-based assessment tools to develop and implement their plan
- Facilitating health and wellbeing groups to promote connections and reduce social isolation
- Maintaining excellent documentation and client records
- Conducting regular reviews to assess outcomes and barriers

**Requirements**
To be successful in this role, you will need:
- Qualifications in Social Work or a related discipline
- At least 3 years' experience in case management, ideally working with individuals and families affected by trauma
- An understanding of trauma-responsive practice and a goal-focused approach
- Excellent time management and organisational skills
- Ability to work independently and as part of a team
