About the role As a Case Manager in our Family Capacity Building Program, Eurobodalla, you ll be crucial in delivering client-focused and outcome-based services.
Your primary responsibility is to work collaboratively with clients experiencing complex issues within our targeted early intervention service, aiming to reduce risks and create positive change within home environments.
You ll work independently as part of a broader dedicated team, connected daily via Teams and phone.
You will support families in meeting their needs, with a focus on cultural inclusivity and trauma-informed care, to improve the lives of vulnerable children.
What does the day-to-day look like at Anglicare?
Advocate for the needs of Aboriginal people, contributing to cultural case plans.Commit to trauma-informed care under the Sanctuary model, integrating it into your daily work.Foster respectful relationships with stakeholders, government departments, and the community for seamless service deliveryDevelop and implement case plans in collaboration with families and other service providers.Ensure that the voices of clients and families are heard in case planning and goal setting.Work respectfully to ensure that case plan goals are achievable and realistic.Support and engage parents and children attending playgroup What's needed?
Relevant qualifications in the Human Services field (Diploma of Community Services or equivalent; Degree qualifications preferred).
