About the Role

We are seeking a dedicated Career Transition Specialist to support individuals with disability, injury or health conditions as they prepare for a meaningful career. As part of our collaborative team, you will guide people through the job-readiness journey - helping them build confidence, develop skills, and achieve their career goals.

  • Working closely with clients as they plan their next career steps, providing support to find the right job along the way.
  • Build strong relationships to understand each client's goals, strengths, and support needs.
  • Provide tailored coaching and regular check-ins to maintain motivation and progress.
  • Connect participants with upskilling opportunities, hands-on experience, and group-based activities.
  • Collaborate with employers and community services to open pathways to employment and training.
  • Meet or exceed internal and external performance benchmarks.
  • Champion person-centred practice and empower individuals to reach their full potential.

About You

We're looking for someone who brings empathy, energy and a strong drive to make a difference. With experience or a genuine passion for employment services, you're confident in supporting people, building trust, and guiding them toward meaningful and sustainable careers. You're outcomes-focused, highly organised, and thrive in a fast-paced, people-first environment.

  • Qualifications in Employment or Human Services (e.g. Mental Health, Psychology, Social Work) – desirable
  • Experience in employment services and supporting people with disability, injury or health conditions
  • Proven ability to meet KPIs and create client-focused employment and skills plans
  • Confident in marketing clients to employers and facilitating group programs
  • Strong communication, problem-solving, and stakeholder engagement skills
  • Knowledge of disability legislation, support strategies, and labour market trends
  • Current Driver's License - essential
