Occupational Therapy Specialist

Key Responsibilities- Provide goal-oriented, time-limited therapy for clients and complete discharge planning in a timely manner.
- Collaborate with the multidisciplinary team to deliver therapy services.
- Driving license required.
- Experience: at least 2 years in occupational therapy.
The Transition Care Program is an initiative supporting older Australians in regaining physical and psychosocial function following a hospital stay. It offers short-term restorative care aimed at enhancing independence and avoiding premature entry into residential aged care.
