Job Title: Rehabilitation Transition Specialist

We are seeking a highly skilled Rehabilitation Transition Specialist to join our team. In this role, you will work with clients to support their journey from rehabilitation to healing.

Your primary focus will be on developing and implementing holistic rehabilitation plans that cater to each client's unique needs. This may involve collaborating with multidisciplinary teams, including medical specialists, other treatment providers, and the employee who has suffered injury, illness, or trauma.

To be successful in this role, you will need to have excellent clinical reasoning skills, as well as the ability to engage effectively with stakeholders across various levels of healthcare. Your passion for helping people rehabilitate will drive your commitment to delivering high-quality case management services.

Key requirements for this position include:

  • Certification or registration with the relevant industry body (e.g., AHPRA, ESSA)
  • Tertiary qualifications in Occupational Therapy, Physiotherapy, Exercise Physiology, Osteopathy, Social Work, Rehabilitation Counselling, or Nursing
  • A minimum of 12 months' experience in a registered profession, demonstrating clinical reasoning and expertise
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, stakeholders, and multidisciplinary teams
  • Occupational Rehabilitation experience is highly desirable
