Care Manager for Complex Needs

Empowering Complex Needs is a dynamic role that requires a skilled and compassionate professional to support individuals with disabilities in achieving their goals outlined in their NDIS plans.

The ideal candidate will have previous experience as an NDIS Specialist Support Coordinator, a Diploma of Community Services or other relevant qualification, Australian PR/Citizen status, and a demonstrated understanding of the complex needs and issues of people living with disabilities and their carers.

We are seeking someone with excellent written and verbal communication skills, computer literacy, and valid Clearance Checks. A valid Driver License is also required for this position.

  1. This role involves working closely with participants to empower them to direct their lives, building a strong network of formal and informal supports, guiding them through complex service delivery systems, and ensuring individual's plan is delivered within allocated resources.
  2. The successful candidate will be responsible for assisting participants in managing their budgets, identifying their needs, and developing strategies to meet those needs.
  3. To succeed in this role, you will need to demonstrate your ability to work independently, think critically, and make sound decisions.
