This is a part-time position within an Intensive Housing Support Team. As a Lead Practitioner, you will work closely with individuals who have complex support needs, providing practical and emotional assistance to help them achieve their personal outcomes in daily life.

The role may involve supporting individuals to maintain safe living arrangements, participate in meaningful activities, and develop healthy relationships.

Key Responsibilities
