Lead Practitioner Role

We are seeking an experienced Lead Practitioner to join our team. In this role, you will be responsible for providing practical and emotional support to individuals with complex needs who are experiencing or at risk of homelessness.

Key Responsibilities
  • Provide person-centred support to individuals with a wide range of needs.
  • Develop and implement individualised support plans to achieve positive outcomes.
  • Work collaboratively with other agencies and professionals to provide comprehensive support.
  • Conduct regular reviews and assessments to ensure the effectiveness of support plans.
Required Skills and Qualifications
  • Degree in Social Work or related field.
  • Post-qualifying experience in supporting individuals with complex needs.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
