Job Description

We are seeking a Support Coordinator to assist our NDIS participants in implementing their plans and increasing their capacity to manage/direct their own supports.

The successful candidate will have the opportunity to:

  • Assess and select mainstream, community, informal, and provider options suitable to each participant's needs and funding restraints.
  • Negotiate services with providers and develop service agreements.
  • Arrange for assessments to determine the nature and type of funding required.
  • Determine budgets for each support type and advise plan managers.

Key responsibilities include:

  • Developing strong relationships with customers, families, carers, advocates, NDIA, and other service providers.
  • Collaborating with Area Managers, Supported Living Managers, and other Support Coordinators.
  • Sharing knowledge and experience with colleagues and members of the Customer Engagement team.
  • Working with Admin, Finance, and Shared Services employees.

Requirements include:

  • A commitment to the Code of Conduct of Coordinated Care Services.
  • Strong interpersonal and active listening skills.
  • Good written and verbal communication skills.
  • Ability to adapt communication style to meet people's needs.
  • Conflict resolution skills.
  • High level of numeracy and literacy.
  • Strong administrative skills.
  • Excellent computer literacy and confidence in using systems and technology.

This is a hybrid working model, with a requirement to present in the office and in the field. Training will be provided to use software packages for file notes and timesheet purposes.
