Care Coordinator for Children and Families

The Allied Health Professional will work in the CYMHS Head to Health kids team, supporting the establishment of a comprehensive mental health service for children and families.
- Assess client needs and develop individualized care plans
- Deliver evidence-based interventions to promote positive outcomes
- Provide high-quality clinical services through thorough assessment and recovery-focused case management
- Offer treatment options for clients with complex mental health issues
- Collaborate with multidisciplinary teams to achieve shared goals
