Job Description

We are seeking a skilled professional to fill the position of Assessment and Transition Coordinator. The successful candidate will be responsible for coordinating the delivery of the Corrections Victoria Reintegration pathway in a prison environment.

Responsibilities
  • Support the development, coordination, and delivery of a range of programs and services across several specialist portfolios, including transition and reintegration, family engagement, disability, diversity, and complex care.
  • Administer prisoner risk and needs assessments, transition and discharge planning, parole application processes, and referrals to pre and post release services.
  • Work collaboratively with multidisciplinary teams to develop integrated and effective transition and support plans.
Requirements
  • Demonstrated understanding of transition and reintegration issues and needs of prisoners.
  • Able to administer risk and/or welfare assessment tools.
  • Effective stakeholder management skills.
  • Ability to communicate sensitively and effectively with diverse cohorts, especially Aboriginal and Torres Strait Islander communities.
Preferred Qualifications
  • Tertiary qualification in an appropriate discipline.
  • Skills and experience in human services, welfare practice, or related fields.
