This role as a Therapeutic Care Case Worker is pivotal in delivering high-quality casework services that focus on effective planning, implementation, monitoring, and review of case plans to achieve permanency and identified outcomes for young people and their families.
The successful candidate will be responsible for providing therapeutic casework services within a trauma-informed framework, working collaboratively with the team to implement strategies and plans that address presenting issues, facilitate the development of new skills and perspectives, promote positive behavioral outcomes, and increase engagement in education, employment, training, and community activities.
Responsibilities- Develop and implement individualized case plans that prioritize the needs and goals of each young person.
- Conduct regular assessments and evaluations to monitor progress and adjust plans accordingly.
- Collaborate with internal and external stakeholders, including ITC Coordinators, Therapeutic Specialists, House Managers, and other relevant professionals, to ensure a holistic response to the needs of each young person.
- Engage in ongoing analysis, decision-making, and record-keeping to ensure the highest standards of care and accountability.
- Bachelor's degree in social work, youth work, community services, or a related field.
- Proven experience working with young people who have complex trauma and an understanding of adolescent development, trauma, attachment, and resilience theory.
- Demonstrated skills and experience in casework practice, preferably in OOHC or child protection, with a track record of achieving positive outcomes for vulnerable young people and their families.
- Knowledge and understanding of Aboriginal culture and values, as well as an awareness of the current issues faced by Aboriginal children, young people, and their families.
