The Disability Support Practitioner will be responsible for supporting clients to develop and maintain independence while advocating for their individual needs.
- Providing personalized care, including mealtime assistance, personal care, medication administration, and manual handling using various equipment.
- Transporting clients using wheelchair-accessible vans and pool cars.
- Monitoring client well-being and reporting progress.
