Care Coordinator for Complex Cases

About the Role:
We are seeking a highly skilled Care Coordinator to join our team in Newcastle. As a key member of our multidisciplinary team, you will provide person-centered care coordination support to children and young people, working collaboratively with families, significant others, and relevant stakeholders.
Key Responsibilities:
• Provide comprehensive care coordination support to children and young people
• Work closely with families, significant others, and relevant stakeholders to develop and implement individualized plans
• Collaborate with multidisciplinary teams and partner with other services to ensure seamless care delivery
• Apply culturally sensitive care coordination practices with Aboriginal and culturally diverse communities
Requirements:
• Bachelor's degree in Social Work, Social Welfare, Psychology, Nursing, or Mental Health
• Experience in care coordination within the community/social sector
• Strong knowledge and demonstrated experience working within relevant legislation, OOHC standards, policies, and procedures
• Ability to work flexible hours and vaccination against Covid-19 required
